On the Watertown Seal is a picture of an English Colonist and an Indian exchanging, as peace tokens,  bread for fish.  Captain Roger Clap landed at Nantasket Point in 1630 and rode up Charles River to Gerry's Landing with the first party of Watertown Colonists.

Print the above forms, fill them out as completely as you can, and bring them to the:
Town Clerk's Office
Administration Building
149 Main Street
Watertown, MA 02472

These forms are not official until they are signed and certified in the Clerk's Office. Other required forms are available at the Clerk's Office.

What You Need

  • Application Form (three copies)
  • Annual Report Form (three copies)
  • One Green Form
  • One Tax Form
  • First-time applicants must submit a statement of organization or bylaws

Forms must be approved and signed by the Director of Police Services (Police Chief). All completed forms must be returned to the Town Clerk’s Office for signature and issuance of permit. Cost is $10. The permit expires one year from date of issuance.

Applications
  • One copy remains in Town Clerk’s Office
  • One copy is for the applicant
  • The clerk's office mails one copy to the state

Annual Reports
  • One copy remains in Town Clerk’s Office
  • One copy to applicant
  • The clerk's office mails one copy to the state

Forms
Forms must be picked up at the Clerk's Office. The Clerk's Office certifies and send a copy to the state.
